The Government, by resolution of May 16, 2024 No. 559, approved the Procedure for registration (creation) and issuance of a military registration document for conscripts, those liable for military service and reservists, as well as a new form of a military registration document.

The text of the relevant resolution was published on the Cabinet of Ministers website; it comes into force today, May 18. Military registration documents drawn up in this resolution are considered valid until a new document is issued.
According to the resolution, a military registration document is drawn up and issued:
- in electronic form – using the Electronic Office of a person liable for military service, the State web portal of electronic public services in the field of national security and defense and the “Action” portal, including using a mobile application (for technical implementation);
- in paper form – on a form, the form of which is approved by this resolution. A person can have only one military registration document on the form.
The information specified in the military registration document of a citizen of Ukraine who is or has been excluded from military registration must correspond to the information contained in the Unified State Register of Conscripts, Persons liable for Military Service and Reservists, otherwise it is considered invalid.
The updated military ID will include;
- Date of Birth;
- last name, first name and patronymic;
- taxpayer card registration number;
- military rank, military specialty and VLK results;
- data on deferment from mobilization
The number of the military registration document on the form and the date of its issue are entered into the Unified State Register of Conscripts, Persons liable for Military Service and Reservists on the day of issue.
