The Russians showed an explicit lack of coordination between the different types of troops and an inability to work on communications at the level of soldiers and unit commanders. They are given a means of secure communication, but not everyone works, and no one knows how to use them. It is not tested during exercises, in peacetime there is a mobile phone that you can always use. And it turns out that the headquarters, which is in the background, does not know where the soldier is and what his circumstances are now. And the soldier doesn’t get a command of what to do next.
Low quality equipment and supplies
