A US Secret Service investigation has found gaps in communication and a lack of diligence by agents before the July 13 assassination attempt on Donald Trump.

Overconfidence by some agents led to a breach of security protocols, said acting Secret Service Director Ronald Rowe.
He added that the guilty employees will be held accountable.
Republican presidential candidate Donald Trump was shot at least five times at a campaign rally in Butler, Pennsylvania, on July 13. A bullet pierced his ear, killing a 50-year-old attendee and wounding two others.
The incident raised questions about the failures of the Secret Service, which is supposed to be keeping Trump under intense protection. Eyewitnesses to the assassination attempt claimed to have seen a man with a rifle on a rooftop near the rally, and warned police and security about it, but they ignored the warnings. Secret Service Director Kimberly Cheatle later resigned over the incident.
